Unfortunately we are not accepting any CHP , Colorado Access, TriCare, or any new Medicaid patients at this time.
If you do not see your health insurance on our list, please call your health insurance and/or our office to determine if we are an in-network provider.
**If your insurance requires a referral for a visit to a specialist, please make sure you obtain one prior to your appointment time. If you are unsure whether or not you need to obtain a referral, please contact your insurance company. Failure to obtain the necessary referrral could result in charges that your insurance company will not pay, and will ultimately be your responsibility.
